Changing Futures in East Sussex
The Changing Futures service delivery team in East Sussex provides support to people experiencing multiple disadvantage through a multi-disciplinary team based within East Sussex County Council’s Adult Social Care department.

The multi-disciplinary team consists of outreach professionals with expertise in areas such as social work, substance misuse recovery, housing, nursing, women’s health, and peer support hosted by our Partner organisations. The team work holistically and intensively with a small caseload of 8-10 clients offering a flexible, person-centred, trauma informed approach, building and nurturing relationships and putting clients at the centre of their own care and support. The team advocates on behalf of our clients to get their voices heard and coordinate multi-agency support where needed.​

The way we work...
We know that working with and supporting people who experience multiple disadvantage requires a holistic approach where time and space is needed to develop relationships built on trust and enable clients to feel empowered. To achieve this, our team spend a lot of time with their clients and collaborate with them to help them achieve their own goals, supported by other specialists within the team depending on the client’s needs.
